ABSTRACT

During the reign of Japanese Government-General of Korea (Joseon) from 1910 to 1945, the main health professionals who were educated about modern medicine were categorized into physicians, dentists, pharmacists, midwives, and nurses. They were clearly distinguished from traditional health professionals. The regulations on new health professionals were enacted, and the licensing system was enforced in earnest. There were two kinds of licensing systems: the license without examination through an educational institution and the license with the national examination. The Japanese Government-General of Korea (Joseon) combined education with a national examination system to produce a large number of health professionals rapidly; however, it was insufficient to fulfill the increasing demand for health services. Therefore, the government eased the examination several times and focused on quantitative expansion of the health professions. The proportion of professionals licensed through national examination had increased. This system had produced the maximum number of available professionals at low cost. Furthermore, this system was significant in three respects: first, the establishment of the framework of the national licensing examination still used today for health professionals; second, the protection of people from the poor practices of unqualified practitioners; and third, the standardization of the quality of health.

ABSTRACT

Chronic osteomyelitis is often considered difficult to treat and may lead to refratory condition in oral and maxillofacial region. Clinical features of chronic osteomyelitis includes pain, swelling, pus discharge, and radiographic change. There are many kinds of treatment of chronic osteomyelitis. One of the most important factors in treating osteomyelitis are removal of infectious foci and administration of massive antibiotics. Recently we reviewed 29 patients with chronic osteomyelitis who visited in our department and treated successfully with our treatment protocols, consisting of surgical intervention and intravenous antibiotics for 2 weeks, and followed by oral antibiotics for 6 weeks. It is concluded that combination of surgical intervention and antibiotic therapy are sufficient to treat the chronic osteomyelitis in oral and maxillofacial region.

ABSTRACT

A review of the literature, provided by a MEDLINE search from 1980 through June 1999, was performed. This study was screened that 649 patients received 679 sinus lift grafts in which 2056 implants were placed. The types of grafts in sinus augmentation were autogenous bone, corticocancellous block bone, allogenic bone, and a variety of alloplastic materials. Results of these grafts are presented. The most frequent complications was the infection of maxillary sinus. Long-term follow-up is necessary to advance the sinus elevation and to support posterior maxillary restorations.

ABSTRACT

The purpose of this retrospective study was to evaluate the treatment outcome of proximal tibial graft. Thirty-nine patients were grafted with proximal tibia for the repair of maxillofacial defect. Complications can be considered incidental and neglectable clinical significance. It is concluded that the proximal tibial metaphysis offers a promising alternative sources of cancellous bone for the repair of maxillofacial defect.

ABSTRACT

During ideal physiologic occlusion in the intercuspal position, all molars are occluded simultaneously and each occlusal contact evenly transmits similar pressure. The purpose of this study is to analyze the occlusion in the mandibular bone fracture patients treated by open reduction. This patients were selected according to the following criteria: 1) complete dentition without any missing teeth, 2) relatively normal occlusion without original TMJ disorder. Occlusal analysis using T-scan system were performed in 22 patients and 20 normal subjects. Our results were followed: 1. The number of occlusal contacts and the magnitude or occlusal force were increased with time. (p0.05).

ABSTRACT

Recurrent epistaxis is not one of rare symptoms in children, the well-known causes of which are anatomical abnormalities of nasal cavity and systemic bleeding tendency. but, in the majority of cases of recurrent epistaxis, it is usually very difficult to find out their underlying causes, so that the treatment is only symptomatic control of nasal bleeding whenever epistaxis occurs, but it usually is impossible to manage against their underlying causes as a specific therapy. The authors considered the breakdown or weakness of vascular integrity of nasal capillary vessel wall as an important factor of recurrent epistaxis, and vitamin C, which has an important role for the synthesis of collagen fiber, could influence to the vascular integrity of nasal capillary vessels. To elucidate the relation between recurrent epistaxis and the status of vitamin C in the tissue of the patients, the authors performed urinary vitamin C loading test measuring urinary excretion of vitamin C with high-performance liquid chromatography by Sirota et al in 19 cases of control group and 32 cases of patients with recurrent epistaxis, whose underlying diseases were not identified. And also the authors administered 1.0 gm/day of vitamin C orally for the treatment of recurrent epistaxis and follow-up was done over 1 year from the beginning of treatment to evaluate the treatment response. The results were as follows: 1) The hemoglobin level was significantly decreased in patient group compared with that of control group (12.4+/-0.8 vs 9.0+/-3.2 gm/dl). 2) The results of urine loading test of vitamin C, expressed as the percent excreted ratio, revealed significantly decreased in the patient group compared to that of the control group (9.1+/-6.1% vs 13.6+/-7.9%). 3) After treatment with 1.0 gm/day of vitamin C orally, follow-up was possible in 23 cases of total 32 patients. Of the 23 patients, "excellent" therapeutic response were in 16 cases (69.6%), "good" response in 5 cases (21.7%), and "no response" only in 2 cases (8.7%). The overall treatment response were in 21 cases (91.3%). In conclusion, it seems that majority of the patients with idiopathic recurrent epistaxis in the deficient state tissue vitamin C so that administration of vitamin C will be one of the effective therapy.

The diagnosis of Cushing's syndrome(key word) associated with hypertensive encephalopathy(key word) due to right adrenocortical carcinoma(key word) was made by clinical features, biochemical studies, radiological studies and pathological examination. The tumor was successfully resected by right adrenalectomy. But 22 months later after operation, he died of dyspnea and heart failure. A brief review of literature was presented.

ABSTRACT

We have experienced a case of Di Guglielmo Syndrome in a 15 years old girl who had the cheif complaints of dizziness, gereral malaise and fine pustules around the nose. It is a systemic hemopathy characterized by abnormal proliferation of defective erythroid and myeloid cells and is a rare disease in childhood. The peripheral blood showed many rubriblasts, myeloblasts, metamyelocytes and bone marrow also showed atypical prorubricytes and bizzar multinucleated rubriblasts. Brief review of related literatures was made.

ABSTRACT

A case of tubular duplicatio of the ileum in a 2 months old infant was presented. This patient was admitted because of abdominal distention and poor general condition. Operation revealed long tubular duplicated blind intestine, measuring 57X5cm in size. The pathologic diagnosis confirmed duplication of the ileum. The patient was discharged without any complication on the 17 th postoperative day. A brief review of literature was made.
